DESCRIPTION
Personalised care roles encompass various professionals who facilitate social prescribing and similar interventions. These individuals dedicate their time to understanding individuals’ needs, focusing on their priorities, and adopting a holistic approach to enhance their health and wellbeing. They link individuals with community groups and essential services to provide both practical and emotional support.If your role involves aspects similar to these, we invite you to participate in our online community of practice forum.
